As many of you know, the treatment of persons who have suffered a traumatic brain, spinal cord and/or orthopedic injury involves a multi-disciplinary approach. Case managers and discharge planners are particularly important because they lay out discharge plans; coordinating the services and resources which seriously injured patients will need upon discharge from the hospital or rehabilitation facility.
